Have you considered AWS?

Your rating: None

Depending upon your security requirements and scale, it may be easy enough to stand up a proxy on Amazon EC2.

We have a few customers who start with a lightweight t2.micro instance running Squid and scale up their instance from there. The advantage of this is:

1) Pricing will be hard to beat. You're only paying for server resources, and you can scale it as needed.

2) Again, depending upon your requirements, there may not be much involved in setting up and maintaining the proxy yourself.

3) You can geographically locate the EC2 instance near your users, reducing network lag.

I'm sure that there are some outstanding hosted proxy providers out there, but due to lack of experience, I can't recommend one myself.
